What Actually Changes in Your Heart and Arteries During Perimenopause

We often hear that heart risk "goes up after menopause," as if a switch flips when periods stop. The research tells a more precise — and more actionable — story. Cholesterol: a sharp, timed jump

The most striking finding concerns cholesterol. SWAN documented sharp increases in total cholesterol, LDL ("bad") cholesterol, and apolipoprotein B within a one-year window surrounding the final menstrual period. This isn't a slow lifelong drift — it's a relatively abrupt rise concentrated in the late perimenopausal and early post-menopausal phases.

Why it matters: that menopause-related acceleration in LDL was later linked to a greater risk of carotid plaque — fatty buildup in the neck arteries that feeds into stroke and heart disease risk. The cholesterol jump during the transition leaves a measurable mark on the arteries years later.

"Good" cholesterol that isn't as good as it looks
