The encouraging truth about bone and muscle in perimenopause is that they respond to what you do. Bone is living tissue that strengthens when you load it; muscle rebuilds when you challenge it. The evidence points to a short list of high-value actions — and most of them protect bones, joints, and muscle at the same time.
Load your bones: weight-bearing and resistance exercise
This is the core intervention. Weight-bearing and resistance exercise mechanically stress bone, which signals it to rebuild — and exercise remains the central factor in managing bone health after menopause, with the clearest benefit at the sites that matter most (hip and spine).1 The same strength work defends the muscle mass that estrogen decline erodes, since estrogen normally helps muscle repair itself.2 Two to three resistance sessions a week is a common target.
Feed the system: calcium, vitamin D, and protein
